Why is Wellington's office looking into the error?

The clerk has been placed on administrative leave. The board of elections will meet Monday to discuss any further disciplinary action. The Mahoning County Sheriff's Office is also investigating.

Someone goofed when placing this issue on the ballot.  The ballot language indicates that this tax will generate new revenues.

The ballot language for the library levy is also incorrect.

The sales tax is one-half of one percent and is for a continuous period of time.
