From the news yesterday

The engine of a 777 disintegrated shortly after taking off from an airport in Colorado for a flight to Hawaii.  The damaged airplane returned for a safe landing at the airport.

What if this happened in the middle of the flight over the Pacific Ocean?  That's flying about 2,500 miles over water.
