Drive (or Don't drive) on Wood Street between Fifth and Wick... you feel like your car is going to shake apart.

Not sure whether it's Northside or Downtown-Andrews Ave from Federal Plaza St northward-all lanes.  We had to go to a custody hearing at Juvenile Court and my mother-in-law commented on the horrible state of a road that led to courthouse.  From what I remember it has always looked and felt that way.

My candidate for the worst section of roadway on the north side is the east bound Madison Avenue Expressway ramp at Wick Avenue.   The ramp has so many patches it feels like driving on a washboard.
